A single 2012 R2 guest resulting in 'Triple Fault' and fails to start

I have an old IBM x3650 (7979) that I use for my dev lab running Server 2012 R2 Hyper-V role.

I have several VM's including Linux and they all were booting fine. I restarted the host server the other day and since then there is a single Server 2012 R2 guest that results in a :

Event ID 18560 : 'MED-WIN-V01' was reset because an unrecoverable error occurred on a virtual processor that caused a triple fault. If the problem persists,contact Product Support.

I've seen a lot of posts relating to BIOS power options causing issues with running multiple VM's, like this HERE but it doesn't seem to be the same issue. I have the power options mentioned already disabled and I'm able to run multiple VM's (including Linux machines).

It's just this one VM that refuses to start. Funny thing is they have all been running for a few months now with no issues, through restarts and all.

The VM itself is not particularly special, dual vCPU, dynamic RAM (same as the other VM's). 

Can anyone offer any insight



VSS Error on CSV volume

Hello, 

For some reason, my w2k12r2 server kept on getting this weird VSS error. 

Volume Shadow Copy Service error: Unexpected error calling routine CLSIDFromString({00000001-0000-0000-0001-000000000001}).  hr = 0x80070002, The system cannot find the file specified.

Operation:
   Obtain a callable interface for this provider
   Check If Volume Is Supported by Provider

Context:
   Provider ID: {26d02d81-6aac-4275-8504-b9c6edc5261d}
   Class ID: {00000000-0000-0000-0000-000000000000}
   Snapshot Context: 0

When I click on the volume, it took me to my CSV volume.  My backup using Veeam for Hyper-v vm would fail. 

I did vssadmin to list writers, there were no errors.   I tried to use Windows server backup to backup my VMs, and it would also fail as well.  Any thoughts ?

Hi i have the following windows 2012 r2 setup

Hv1 - hyper-v installed with failover clustering on SAS- hosting virtual DC on SAN

hv2 - Hyper-v installed with FC + DC on sas hv3 Hyper v + FC on SAS

When running the failover validation wizard i get the following message for all 3 nodes HV1 , Hv2 , hv3?

"The service principal names (SPNs) of x.x.com could not be determined because of this error: The specified domain either does not exist or could not be contacted"

i have the following windows 2012 r2 setup:

Hv1 - hyper-v installed with failover clustering on SAS- hosting virtual DC on SAN

hv2 - Hyper-v installed with FC + DC on sas

hv3 Hyper v + FC on SAS

When running the failover clustering wizard i get the following on each node:

Any ideas please?

Unable to see previous VM name in DPM Protection Group in Online State.

Hi,

Suppose i am having the VM named as VM_XYZ hosted on Hyper-V. I took the backup of .vhd file related to XYZ machine into the external drive and then i created the new VM into the same Hyper-V named asVM_XYZ-NEW with the same configuration and setting, after that i used to attached the .vhd file of  previous (VM_XYZ) vm to new name(VM_XYZ-NEW) vm.

In between i deleted the previous VM (VM_XYZ)from Hyper-V because it does not have it .vhd file.

Now in our environment  DPM (Data Protection Manager ) is responsible to take the backup of all the VM to Tape Library. In Past
we created the Protection Group and added all the machines into it.Previously DPM is taking the backup of  all the virtual machines includingVM_XYZ  smoothly . we added this new VM into VM_XYZ-NEW into that protection group and DPM is taking the backup smoothly.

But i need to change the VM name as its previous one (i.e VM_XYZ), when i m going to do this DPM does not showing my virtual machine  in protection group intoONLINE state, it is showing in OFFLINE state. It too important to change the name of my VM from previous to new one (i.e VM_XYZ-NEW to VM_XYZ).

What will i do to eradicate this issue.??? Please suggest.


 

PXE-Boot with Hyper-V based on W2K12 R2 with Gen2-VMs

Dear Community,

as you probably know the new R2-Version was advertised with the ability of PxE-booting with the synthetic instead of the legacy-nic which works with poor 100 mbit/s.

Here´s the problem:

When I create a new virtual machine with Gen2 without the option "Secure Boot" and PxE-boot it, I get the following error:

Boot failed. EFI Network.
Boot failed. EFI SCSI Device.
No Operating System was Loaded. Press a key to retry the boot sequence

When I create another new virtual machine with Gen2 but with the option "Secure Boot" and PxE-boot it, I get this error:

Station IP address is x.x.x.x
Server IP address is x.x.x.x
NBP filename is pxelinux.0
NBP filesize is 15888 Bytes
Downloading NBP file...
Successfully downloaded NBP file.
Boot Failed. EFI Network. Failed Secure Boot Verification.
Boot Failed. EFI SCSI Device
No Operating System was Loaded. Press a key to retry the boot sequence

When taking a look at the log of the TFTP-Server (TFTPD32) I get the same information regardless which way I do it. The only interesting thing it says is:

"Peer returns ERROR <User aborted the transfer> ->aborting transfer"

I have also tried to use another tool called Serva as TFTP/PXE-Server -> The error still occurs.

Before anybody wants me to try to do it as Gen1 -> I know that this works, but I want to get it running under Gen2 since the synthetic one works (should work) much faster than the legacy-one.

Really looking forward to get some useful information here.

Hyper V server can not be accessed via RDP directly

Our Hyper V server is not accessible via RDP from the domain admin credentials. But i was able to have RDP access when i tried with LOCAL ADMIN credentials. Here are the observations.

1. We had to face an unexpected power failure and the hyper V server went  down. We are experiencing this issue after that incident.

2. Hyper-V server is in the domain without any problem.

3. I have checked the RDP allowed user list. (Domain admin group is available)

4. Previously it was working without any problem.

It would be great if someone can guide me to troubleshoot this issue.

Update and Reboot of Hyper-V servers

Hi,

We would like to get info or Microsoft recommendation about hyper-v update. How often is best practice to update because we need to manually turn off VM's (for security reasons and to avoid problems) and reboot hyper-v and that costs our clients a lot. We are updating our servers through Kaseya and we cannot reboot Hyper-V servers every week because of one update (sometimes). 

Hope that someone can share with me best practice or what we can do?

Move VHD from one VM to another

I have VMs running on Hyper-V 2008R2 in cluster. One VM is running Win2008R2 with two VHD's attached. One of them is a file share only that I need to move to another VM running Win2012R2. If I move that VHD over to the new VM will it also move the NTFS rights/share permissions? It's very large so I really don't want to copy from one VM to another.

Hyper-V - MPIO configuration on Fiber Channel

Hi Everyone,

I'm building a four-node Hyper-V 2012 R2 Cluster. Is there any special configuration for MPIO when using Fiber Channel SAN? Do I need to just install MPIO feature? What are the procedures for that (for hyper-v hosts to be able to correctly detect LUNs)?

Migrate Virtual Server VM w/ SCSI disk to Hyper-V IDE

Ok, i'll start out what I'm trying to do. Currently I have a Virtual Machine running on Virtual Server 2005.  Now what I would like to accomplish is to migrate this VM to my Hyper-V server. Now for most of my VM's this was pretty seamless, uninstall the old Virtual Machine Addons, turn off the the VM, move the VHD, create the new VM in Hyper-V, install the new addons, and i'm good.  However, now i have 1 machine left to move, however, its not an easy move. 

When i move it over and start up the hyperv machine i get a STOP 0x0000007B error.
If I put in the windows disk to do a repair on it, when it gets to the part where it detects existing Windows Installs, both partitions come up as "damaged or corrupted partition".

I think the root of the cause is that it is running a SCSI adapter to connect to the VHD. Now the other side of this is that the VM OS is windows 2000.

Host machine is a Windows 2003 R2 x32 Virtual Server.
Now the VM is setup as:
1 x SCSI Adapter
1 x 1 VHD File (attached and booting to SCSI Adapter)
OS Disk Type: Dynamic  (legacy from P2V migration)
OS: Win 2004 SP4 SR1

Now my time window to get this moved over isn't great as this is a critical server (yea, i know its only running windows 2000 and it does need to be upgraded but thats another issue that i'm dealing with).

So what I've tried is:
 - Straight VHD Copy and build a new machine - Stop 0x07B
 - Imaged machine with Acronis Echo Server - Did the P2V Conversion and created a new VHD - Stop 0x7B
 - On the hyperv (vhd connected with IDE) Started windows 2000 off the cd, it detects my partitions as damaged or corrupted.

So I guess my questions is:
 - Is there a difference between a SCSI VHD and an IDE VHD?
 - Can i convert SCSI to IDE?
 - Should I install an IDE controller on my Virtual Server VM. Shut it down, then copy it over and attach it to an IDE controller on the HyperV? Or should i try attaching this to the IDE channel then booting it up on Virtual Server? Is this an option?
 - Or do I install a fresh copy of windows 2000, then do the system backup and system state backup to bring the files over? (I'm hesitant on this as there are automatic processes that connect to the server and update things. So inorder to do a backup, I need to have it connected to the network (which will potential have processes trying to update it).  I guess i could do a backup to another VHD and take the thing off the network that way.
      - If i do a system backup to copy the system state over, does it matter that i'm moving from scsi to ide? Will this break?



Hyper-V Replica cancellation

Hi,  I hope you can help with an urgent query.  In an environment running Hyper-V replica for a handful of servers between 2 physical hosts - HOST1 went down, and an Unplanned Failover was initiated to HOST2.  Once HOST1 comes back up the servers can be failed back to the original host.  However, how would you best fail back a virtual server running Exchange 2010 on the Replica server?  I know the scenario is not support by Microsoft, and this should never be part of a replica, however what is now the best way to fail-back this Exchange VM onto HOST1 without causing problems?  The options I can see are:

1. As per any other server, use the "Reverse Replication" option - I am not confident that this is likely to succed given the lack of support for Exchange, but could this be the simple fix that allow the Replica to copy its changes back to the original VM and start up without problem?

2. Back up any emails received since this failed over, use 'Cancel Replication' in Hyper-V to 'undo' any changes since this morning & simply fire up the original version of the Exchange VM. We could then restore the emails recieved this morning.  Given that the remaining servers including Active Directory DCs etc are currently running alongside the failed-over version of Exchange VM, will this cause problems when bringing the original Exchange VM back up on HOST1?  Effectively AD will think Exchange has been running all morning but the Exchange server will believe its just powered back on.

Host and Guest - 2 external IP's

Hi all,

At the moment I have a server that has an external IP address on it, and the datacenter has given me another IP for it. The host OS is Windows 2012 R2 Datacenter. 

I also want to have another Server 2k12 R2 guest running on it. However, I want to use IP1 on the host and IP2 on the guest. All the times I have done this before it hasnt worked. Does anyone have any documentation to do this? Is it a case that I don't configure the 2nd IP on the host and just assign it to the guest (through the public switch). All the times that I have done this before, it has been a case of either the host or the guest has external access, and not both at the same time, which is what I want. 

Is this possible, if so, How do I do it?

Hyperv-V 2012 R2 Failover Cluster questions

Hi,

I've built a Hyper-V cluster. It all looks good, but just one question I had: If I want to have granular control on which NIC  (and thus IP) is used to communicate to the ISCSI target, how could I do this? In the target settings, I chose the DNS name for the initiators that connect to the target, but the initiators (i.e. Hyper-V nodes) have more than one IP address.

Also, the nodes have 8 NICs but I only see two network connections in Hyper-V Manager?

I have a Windows server 2012 standard running hyper v, on that I have two VMs 1 is  a domain controller for the network it is the only dc on the network. it is also running windows server 2012 standard, the domain controller has recently been losing internet connection, when I restart the dns client service connection returns, there are no events in the event viewer that could explain it. If I try to browse to a website and the run troubleshooter I get the error "website  is online but isn't responding to connection attempts" I can ping web addresses and get a response. The host doesn't lose connection at all it's just the VM.

Hyper V - Loosing external network trafic after creating vswitch

Hi there

I'm running into an issue with a new server.

As soon as I create a vswitch using the physical network interface I'm loosing incoming traffic to this server (rdp, ping...) BUT I can still connect using the HyperV console

From the server itself, there is no network issue (can ping other machine, rdp...)

If I remove the vswitch, everything is going back as expected

I have already check lot of thing (based on some blogs, including MS ones) with no luck. Even did a full reinstall of the OS

Any idea? Please don't tell me the firewall as this is not the issue - firewall rules are applied using GPO and other HyperV server don't have any issue

VM Rebooting every few days

Hello  I have host machine running 2008 R2 with 4 VM's.   The problem is two of the VM's restart every couple of days.  The host was restarted over the weekend and that seemed to help for a bit then it came back.  The only errors I can find on the host about that time is  Source VHDMP ID 2 and  ID 4.  There are no other errors or warnings the hour prior to that.  Any ideas?

Failed to surface VHD D:\Hyper-V\XENAPP\XenApp.vhd. Surface attempt was cancelled.

2012 hyper-v replica page file best practice

Hello,

As I understand the best method of excluding page file and have a good planned failover, is to initially exclude the page file on a separate VHDX, then shut down system, and copy the VHDX with the page file and attach to replica before booting.

1. Is this the best method of getting a replica VM up and running without errors with the page file?

2. Is it ok to leave the page file off completely, and let the replica re-create temporary page file when it boots up, thinking in terms of File Servers, and SQL servers.

Network issue on VM

Hello,

i'm currently installing a third party antivirus product with IDS functionality and all the servers hosted on hyper-v servers (both 2012 and 2012 R2) shows portscan attempts in the firewall logs. I'm trying to find the reason for this behavior and it seems that the VM's nic is act as it was in promiscuos mode and capture packet that are not directed to nor originated from the VM.

Anyone alse saw a similar behavior or have some idea to solve this strange issue?
